Imagine watching your daughter learn to speak, walk, and use her hands — and then watching all of it disappear.
Not gradually. Not subtly. In a period of months, during what should be the most vibrant years of early childhood.

**FDA Breakthrough Therapy Designation**
On **February 26, 2026**, the US Food and Drug Administration granted **Breakthrough Therapy designation** to **NGN-401**, Neurogene's investigational gene therapy for Rett syndrome.
Breakthrough Therapy designation is the FDA's most significant signal of clinical promise. It is granted when a therapy shows *substantial improvement over available therapy* for a serious condition — based on preliminary clinical evidence. It triggers intensive FDA guidance and a faster review pathway. The agency is saying, clearly: *we have seen enough to prioritise this*.
The designation was based on interim Phase 1/2 data through October 2025. The FDA reviewed those results and concluded they demonstrated **clinically meaningful and durable functional improvements across multiple Rett syndrome domains** — including continued skill acquisition over time after treatment.

**How the Therapy Works**
NGN-401 is a **one-time treatment**. A modified adeno-associated virus vector delivers a working copy of the full-length human **MECP2 gene** directly to brain cells — replacing the defective gene that causes Rett syndrome at its source.
The core challenge in MECP2 gene therapy has always been dosing precision. MECP2 expression exists in a narrow therapeutic window: too little, and Rett syndrome persists. Too much — MECP2 overexpression — causes a separate, serious condition called **MECP2 duplication syndrome**. Getting the amount right, cell by cell, across a living brain, has been the central unsolved problem in this field for years.
Neurogene's proprietary **EXACT™ transgene regulation technology** is designed specifically to address this. The system builds a volume control into the gene delivery itself — allowing MECP2 expression at levels that rescue function without triggering toxicity.
**What Patients Are Experiencing**
The interim Phase 1/2 data showed patients **continuing to acquire skills** after receiving NGN-401. Not just stabilising at their pre-treatment level — but actively gaining abilities across multiple functional domains: motor function, communication, breathing patterns, hand use.
The Rett syndrome patient community has described these results with a word rarely used in this disease's history: *unprecedented*.

For the girls in this trial, and the families who watched regression happen and hoped for something more than management — that would not be a milestone.
