Martha's Rule Has Received 10,000 Calls in 16 Months — And Changed Care for Thousands of NHS Patients

A scheme that empowers worried families to request an urgent review of a loved one's hospital care has received over 10,000 calls in its first 16 months — and the results show it's working exactly as intended.

New data from NHS England reveals that one in three calls to Martha's Rule identified a rapid worsening of a patient's condition, triggering 446 potentially life-saving interventions where patients were escalated to more intensive care.

**What Is Martha's Rule?**

Martha's Rule is named after Martha Mills, a 13-year-old who died in 2021 from sepsis after her parents' concerns about her deteriorating condition were not acted upon quickly enough. Her father, Paul Mills, campaigned for a system that would ensure families' voices are heard.

The rule, which launched in April 2024, gives patients and their families 24-hour access to a dedicated phone line and a rapid review by a separate clinical team if they're worried about a patient's condition. It applies to all NHS acute trusts in England.

'This is about recognising that families often know when something is wrong — even when the numbers on a chart look okay,' said Amanda Pritchard, NHS Chief Executive. 'These 10,000 calls represent 10,000 moments when families were heard.'

**The Numbers Tell the Story**

The NHS data breaks down what happened after those 10,000 calls:

• **3,300+ calls** identified rapidly worsening conditions requiring immediate attention • **446 patients** were escalated to critical care or received life-saving interventions • **Thousands more** received reassurance, additional monitoring, or adjustments to their care plan

Critically, the data shows that families' instincts were often right. The scheme has given clinical staff a structured way to respond to family concerns that might otherwise have been overlooked during busy shifts.

**'This Is What We Fought For'**

For Paul and Merope Mills, Martha's parents, the statistics represent something deeply personal.

'Every one of those 446 escalations could have been a family like ours, waiting to be told their child isn't coming home,' said Paul Mills. 'Instead, those families got to have different endings. That's what we fought for.'

The scheme has been particularly effective at catching cases of sepsis — the condition that killed Martha — as well as post-surgical complications and sudden deteriorations that can be hard to spot on routine observations.

**A Model for Others**

Healthcare systems in Australia, Canada, and several European countries are now studying Martha's Rule as a model for patient safety initiatives. The core principle — that families are partners in care, not bystanders — is resonating with healthcare reformers worldwide.

'What makes Martha's Rule powerful is its simplicity,' said Dr. Aidan Fowler, NHS National Director of Patient Safety. 'A phone number. A guarantee of being heard. A rapid response. Sometimes the most effective safety systems are the most straightforward.'

NHS England has announced that the scheme will be expanded to include mental health trusts and community hospitals later this year. 💙🏥
